Back pain is the largest single cause of sickness absence from work and, for many people, can be a chronic problem. It is responsible for 93 million working days lost each year in the UK and is more likely to occur during the ages of 30 to 50, the most productive period of most people’s lives. Back pain is so common in the UK that everyone can expect to suffer from it at some time. Lower back pain

Lower back pain or low back pain is very common and is the single most common chronic pain syndrome seen in pain clinics in the Western world. This pain is often aggravated by poor lifting techniques, where the sufferer relies mostly on the muscles of the lower back rather than those of the legs, to lift or move any given object. This causes a great deal of inconvenience to those who experience it. People experiencing lower back pain are often best served by remaining at work but only a trained certified health professional confident in their assessment skills can push in that direction.

Treatment

Treatment should be directed at the specific cause of pain, it can help to speed things up but many back-pain sufferers don’t seek treatment early enough. Treatment for back pain generally depends on what kind of pain you experience: acute or chronic.

Chronic

Chronic back pain is defined as pain that persists for longer than three months and is much less common than acute pain (sudden/sharp onset). It is the most common cause of activity limitation in persons younger than 45 years. The symptoms of chronic back pain usually arises because there’s an underlying condition such as arthritis or osteoporosis, or because the injury was severe. Chronic back pain is one of the commonest ailments and a cause of private distress, considerable personal and public financial consequences and a condition that is often difficult to treat.

Back pain is a major problem in society, although it is rare in teenagers and not common as a new presentation in the elderly. It is often a recurrent symptom and most patients first experience it in their twenties or thirties. Back pain is now one of the most common reasons for seeking health care, sickness absence from work and long-term receipt of incapacity benefits. Most acute attacks settle quite quickly, usually within a few days or weeks, allowing the sufferer to get back to most ordinary activities.

Disc herniations and disc bulges are very common causes of neck and back pain. When they are acute, some of the symptoms may be numbness and tingling down the arms and into the hands (when the disc issues are in the neck) and down the buttocks, legs and feet (when the disc issues are in the low back). Injuries to the discs in the low back or neck can be caused by anything from a sports injury or car accident to a simple movement or even by sneezing (as was the case with a few patients that I’ve had).

The symptoms of a disc herniation may come and go and at it’s worst, they can be even excruciating and certainly infringe upon a person’s ability to function daily at work or even disturb one’s sleep. There aren’t too many options for treatment of a disc herniation. Usually if the injury is confirmed by a MRI of the neck or low back, the first options are simple medication and/or physical therapy. If that fails, the next level of treatment would be corticosteroid injections at the level of injury, which may offer some slight but, temporary relief. Finally, if all else fails surgery is often recommended.

However, as most spine surgeons will tell you, it’s often hard to predict whether or not you’ll respond to spine surgery and it should be considered only as a last resort. It may be that you do respond well to surgery however, there is the possibility that you will remain the same symptomatically, or your condition gets even worse. The research also shows, if you get more than one spinal surgery, your likelihood of ever making a full recovery diminishes.

As a practicing Chiropractic Physician, I’ve seen my fair share of disc bulges and herniations in my office. Typically, I see patients once they’ve had all the aforementioned conservative treatment (medication, injections and physical therapy) and they are still no better; when they’ve just injured their disc(s) or when they’re referred in for a second opinion. I emphasize utilizing a conservative approach of gentle chiropractic care, massage therapy and acupuncture first followed by, some physical therapy.

If however, they’re not responding to that or they’ve already tried those therapies, I recommend Spinal Decompression. Spinal Decompression is a relatively new therapy that is completely conservative, drug-free and non-surgical. The purpose of the therapy is to provide a negative pressure between your vertebra at the level of your injured disc(s) to pull the herniated back into it’s normal place, allowing it to heal and relieving pressure off of the nerves in the area. The therapy usually lasts approximately, 30 minutes or so and a typical treatment protocol includes exercises and other modalities over the course of several treatments.

While spinal decompression is not for everyone, it is an excellent option for people who are seeking a conservative modality that treats their condition/discs without putting anything into their spines like an injection or pull anything out like a surgery. If you’re suffering from a disc herniation and would like another opinion or treatment option, certainly seek out an office that provides spinal decompression in your area and speak with them about getting a consultation to see if you’re a candidate for care. 3 Common Causes of Lower Back Pain

Low back pain keeps you from enjoying the small things in life such as spending time with your kids, taking your pooch on his daily walk, playing a game of golf with your buds, or just lounging in your favorite chair watching television. Almost every movement you make involves the use of the muscles in your back.

So naturally, when your back is injured, the quality of your life will suffer drastically. Here are some common causes of lower back pain.  

Improper Use of the Back Muscles

The most common causes of back pain are strains and sprains from injuries to the back due to improper use of muscles. Back muscles can inadvertently be injured from engaging in day-to-day activities such as heavy lifting, picking up children, improper bending, toting an oversized purse, slouching in your chair, sitting for long periods of time, and sleeping on a rickety old mattress.

Being overweight can also put extra strain on the muscles in your back and cause lower back pain. The onset of pain from strains and sprains can come immediately, or days later. When the muscles in your back are injured, they will knot-up, or spasm in an effort to prevent more damage. These spasms result in severe low back pain, and difficulty moving.  

Slipped or Herniated Discs

Slipped discs, also known as herniated discs can result from several things including trauma to the back, disc degeneration, aging, sudden twisting movements, excessive pressure on the disk from heavy lifting, and being overweight. A slipped disc in the back puts pressure on the nerves, causes inflammation, and varying degrees of back pain.

A herniated disc can affect more than just the lower back area. If the damaged disc puts pressure on the sciatic nerve, it results in a condition known as sciatica. Sciatica causes burning and needle-like pains that shoot from the lower back, all the way down the distance of the back of the leg.
Osteoarthritis
Osteoarthritis is a degenerative condition that can affect several parts of the body, including the back. This condition doesn’t happen overnight; it gradually causes deterioration of the discs between the vertebrae that cushion the bones. Once these disks deteriorate, the vertebrae will press against each other and cause back pain and discomfort.  

Osteoarthritis is more common in older people because the body changes as you age. Body parts travel south, skin starts to thin, and your bones become more fragile due to calcium loss. This loss of calcium makes your bones brittle and more prone to compression fractures that put you at risk for back pain.  

Strains and sprains from improper muscle use, slipped discs, and osteoarthritis can all cause pain in your back. Thankfully, technological advances and improvements in modern medicine have made it possible for chiropractors to offer their patients several choices to manage or end debilitating back pain for good.

Middle Back Pain And Cancer

Random abdominal pain?

Every few weeks/month I get an annoying problem with my stomach that will last for afew weeks, go away, and then come back.
The pain is usually around the middle of my stomach/sides. It’s a somewhat.. light pain when it hurts, more uncomfortable than anything, but it still really annoys me.
When it does happen to hurt more, sometimes it hurts a little bit when I press in on my stomach.
At the beginning of all of this, my ‘bathroom’ habits change, but it doesn’t seem like constipation to me.

It seems like sometimes yogurt helps it, and the discomfort usually seems to temporarily go away when I eat ((No, the discomfort isn’t because I’m hungry))

I guess my main question here is does it seem like this could be anything serious? Stomach cancer, or some form of cancer, or maybe an Ulcer?

I don’t have the money to go to the doctor at the moment, so any help here would be very appreciated.

you cant afford not to go to the doctors, dont they have indigent care programs in your area? they go by income or free clinics, just go and worry about the money later. ive had surgery twice without ins, its not impossible to get medical care when you need it

Mammograms Are No Joke – They Can Save Lives

There are so many jokes about mammograms! Have you heard the one about the fridge door – or the bookends – or the garage floor? Thanks to all the jokes, “Mammogram” has become a household word, and it’s not that I don’t have a sense of humor, but as a mammography technologist, I’ve heard the jokes many times. I think the jokes are embarrassing for women and demeaning with regard to their physical bodies. Many women say, “If men had to do this, there would be a better solution” – this may or may not be true. Most people agree that mammograms are not perfect, but until there is a better solution, I think it’s time to look at mammograms in a different light.

In May of 1985 and 1986 I asked my doctor to order a mammogram for me and he refused both times saying I was too young. There were no screening mammography centers to which I could refer myself, so that was that. In December of 1986 at the age of 42 I felt a lump in my breast and had a mammogram the same day. It turned out to be Stage II breast cancer with 4 positive lymph nodes. I had a lumpectomy, a mastectomy and chemotherapy but chose not to have radiation. I obviously wasn’t too young to have cancer.

In May 1985 a mammogram cost less than $60.00 and would have resulted in my having minor surgery to deal with a small lump. Delaying the diagnosis until December 1986 raised the cost of the medical care I received both in dollars and the amount of human suffering we faced. I say “we” because a diagnosis of cancer affects the family, friends and community of the person with the disease. A timely mammogram would have saved us all a lot of grief.

The common perception is that having a mammogram is a negative experience; I think this is a bad rap. Mammograms are quick and easy breast X-Rays; which usually means two views of each breast – one from the top and one from the side. They are performed by friendly, knowledgeable technologists who do their best to help women feel at ease. The technologists’ goal is to get the best films possible and also to make the experience as quick and painless as possible.

When people go for a mammogram the most important thing to know is that relaxation of the upper body is the key to a positive experience. I know it’s hard to relax when you’re apprehensive, but this is why I believe we need to lessen the public apprehension of this test. It is easy to relax by taking some deep breaths before you have the test. By relaxing your muscles you will be much more comfortable through the test than if you are tense. An added bonus is that the films will be of higher quality, as it is easier to image the back of the breast close to the chest wall if the pectoralis muscles are relaxed. When it’s done, you may hear yourself saying, “That wasn’t bad at all!”

Some women are embarrassed to have a mammogram because they don’t want anyone other than their partner to see and touch their breasts. The mammogram jokes add to their fear of pain and embarrassment making it harder for them to manage, and I know of some women who avoid having a mammogram for this reason. The test is done in privacy; no one but a female technologist will be present. Technologists, for the most part, are sensitive people who will do the test as quickly and professionally as they can. Many women who have resisted the test for a long time are amazed at how simple and painless it can be.

Mammograms include compression of the breast with a plastic plate to produce a high quality image with the least amount of radiation. Breast compression is meant to be tight, but it should not be painful and it only lasts for a few seconds. If you think about looking at a bunch of grapes – it’s hard to see them all from one spot. If you spread the grapes out, you can see more grapes. Similarly with the use of compression, more breast tissue is visible when the breast is spread out. With a flatter, thinner layer of tissue the amount of radiation required is less than if the breast is not compressed. The amount of radiation you get is as low as can be achieved if adequate compression is used, and also if good quality control is maintained at the mammogram facility.

In the U.S.A. the cost of a mammogram runs between $50 and $150.00. There is financial help available from insurance companies, state and local programs, and from some employers. Please do not let the cost deter you from having a mammogram as the cost of not having a mammogram can be much higher both financially and emotionally. Check for information on the internet.

In most places in Canada, women can book their own appointment for a free screening mammogram; a doctor’s referral is not required. In places without a screening program, mammography is available with a doctor’s referral and is covered by health insurance. Approximately 7% of women will be asked to have further testing.  Most of the time, follow up testing involves an additional mammogram with a different view to separate the breast tissue in a particular area to get a better image.  In my analogy of the bunch of grapes, it’s like having a few grapes on top of each other and separating them out in a different way in order to see them better.

There is controversy about the age bracket for women to have a mammogram. On a mammogram film, normal breast tissue in young women usually appears to be dense; normal breast tissue in older women usually turns to fat and appears less dense. Reading mammograms on young women is like looking through a tree which is full of leaves in summer. Reading mammograms on older women can be compared to looking through a tree in winter. You can see why reading mammograms on young women is more complex than reading films on older women and this is the main reason why screening mammography is more effective as women mature.

The fear of being diagnosed with breast cancer will often prevent a woman from having a mammogram. My personal experience is that it is much better to be diagnosed earlier rather than when the cancer has had chance to spread. The amount of fear, pain, embarrassment, and emotional anguish from having a mammogram does not even come close to that of being diagnosed with an advanced cancer. A mammogram takes about 10 minutes; an early cancer can be dealt with in a reasonable amount of time, while an advanced cancer is much more of a time commitment. The amount of fear that comes with a cancer diagnosis is astronomical compared to that of a screening mammogram.

It is often recommended that women have a screening mammogram every two years, but many people believe it is better to have mammograms on an annual basis. It is probably best if women can consult their doctors and make the decision on an individual basis. A number of factors affect the decision such as age, family history, general health, and previous breast problems. Between appointments, whether you choose to have a mammogram every year or every two years, it is important to be aware of any breast problems. If you notice anything unusual it is wise to contact your doctor.  This applies even if your mammogram was negative because there are a certain percentage of cancers that do not show on a mammogram.

The Canadian Breast Cancer Foundation promotes a three-prong approach to breast health:
* annual clinical breast exam by a doctor or trained health professional
* screening mammogram
* monthly breast self exam

Breast self-exam can be a controversial issue. Many people do not recommend monthly self-exams, yet many women have found their own breast cancers this way. The important thing to remember is if you choose to do self exam, to do it right:
* learn the proper method from a doctor or trained health professional
* be disciplined and practice it regularly
* pick the same time of your menstrual cycle or the same date each month
* get to know your normal breast “architecture”
* make notes of your findings, draw pictures and record dates
* make detailed notes of unusual findings including dates
* check with your doctor if you find anything worrisome

Following these steps will give you confidence and put you in charge of your breast health. Some health professionals are concerned that women will be unnecessarily alarmed if they find a problem with their breasts. I believe that an educated approach to breast care will reduce the fear that many women live with, and they can consult their doctors in a more rational manner. Most breast lumps are benign, but early detection of breast cancer is worth the extra cost of investigating lumps and other unusual findings.

Mammography is a peculiar test in some ways. However, it is the gold standard at present and until there is a better method of screening which is also cost effective it makes good sense to have regular mammograms. Finding cancer in the early stages before it has a chance to spread makes the treatment much easier and the cure rate much higher. Having a mammogram is not meant to be funny, or even fun; but a few minutes of discomfort rewards us with knowing we aretaking action to help protect our breast health.
